When you submit the consultation form, we collect the details you choose to provide: your name, organization, work email, optional phone number, the type of organization, the states where you operate, the services you're interested in, an optional provider count and description, your preferred contact method, and your consent to be contacted. We also record the date and time of your request.
The form is not for sensitive information. Please do not send Social Security numbers, dates of birth, credentialing documents, passwords, patient information or similar data through it.
Information collected during an engagement
If you become a client, we collect only the information needed for the services you purchase, through secure, purpose-specific links. Depending on the service, this may include provider identifiers, licenses and certificates, ownership and location details, and supporting documents. We don't ask for sensitive identifiers such as Social Security numbers or dates of birth on this website or by ordinary email; when a service needs one, we ask for it through a secure link.
How we use information
